Why is ADHD underdiagnosed?
It is commonly under-identified for AFAB people because it looks different. The hyperactivity part is usually internal for people assigned-female-at-birth, who are usually the more inattentive type.
More importantly, the Diagnostical Statistical Manual has always defined mental disorders in terms of dysfunction. But ADHD is a neurotype, not necessarily a disorder, so people who are successful at work and school are not usually diagnosed.
